Operation Immunization is holding an immunization clinic on 11/16 and 11/23 from 9:30 am to 1 pm at Cook’s Pharmacy in Kingston. Along with immunization, we are offering to take blood pressure of patients in the waiting area. There are 1-hour time slots requiring a person to immunize and a person to take blood pressure/ complete paperwork.

The following documentation are required in order to immunize:
– PAPALS Intern License
– PAPALS intern intent to immunization certificate
– APhA immunization certificate
– BLS Certificate
– Blood-borne pathogens certificate
