To qualify, complete\u00a0<strong>2 of 3 approved wellness activities<\/strong>\u00a0by the end of the\u00a0<strong>Open Enrollment period in November<\/strong>\u00a0(exact date TBA).\u00a0<\/p>\n<p>Activities include:<\/p>\n<ul>\n<li>\n<p>Committing to a physical or mental wellness practice<\/p>\n<\/li>\n<li>\n<p>Attending a Wilkes Wellness or HR-approved event<\/p>\n<\/li>\n<li>\n<p><a href=\"https:\/\/docs.google.com\/document\/d\/162nLrzC3gERsaaeuSWdmIo7c3rmHz0Potbg0xPjvVIQ\/edit?usp=sharing\" target=\"_blank\" rel=\"noopener\">Reading one approved wellness article<\/a><\/p>\n<\/li>\n<\/ul>\n<p>Incentives include:<\/p>\n<ul>\n<li>\n<p>Additional\u00a0<strong>HSA contributions<\/strong>\u00a0($250 for single \/ $500 for other tiers)<\/p>\n<\/li>\n<li>\n<p><strong>Payroll contribution reductions<\/strong>\u00a0for those on the PPO $400 plan<\/p>\n<\/li>\n<li>\n<p>$100\/month for employees who waive Wilkes medical coverage<\/p>\n<\/li>\n<\/ul>\n<p>Complete your activities and submit the\u00a0<a href=\"https:\/\/docs.google.com\/forms\/d\/e\/1FAIpQLSfJgcfZP1dHq5ZzVp6HJjqiLzbS1mHzz-LfehEfAUuvMM-fqA\/alreadyresponded\" target=\"_blank\" rel=\"noopener\">Google Form<\/a>.<strong>\u00a0<\/strong><\/p>\n<p>Questions?
