Full-time, benefit-eligible employees who complete approved wellness activities by <strong>November 26, 2025<\/strong>, can earn incentives for the 2026 benefit year!<\/p>\n<p><strong>Incentives include:<\/strong><\/p>\n<ul>\n<li>\n<p>\ud83d\udcb0 <em>For Wilkes Medical Plan participants:<\/em><\/p>\n<ul>\n<li>\n<p>$250 HSA deposit (single coverage) or $500 HSA deposit (family\/other tiers)<\/p>\n<\/li>\n<li>\n<p>Or an equivalent reduction in payroll contributions for PPO $400 plan participants<\/p>\n<\/li>\n<\/ul>\n<\/li>\n<li>\n<p>\ud83d\udcb5 <em>For employees who waive Wilkes Medical Coverage:<\/em><\/p>\n<ul>\n<li>\n<p>$100\/month (annual base pay under $62,000)<\/p>\n<\/li>\n<li>\n<p>$50\/month (annual base pay between $62,000\u2013$100,000)<\/p>\n<\/li>\n<li>\n<p>Proof of alternate coverage required<\/p>\n<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p><strong>How to Participate:<\/strong><br \/>Complete <strong>two<\/strong> of the following by November 26, 2025:<\/p>\n<ul>\n<li>\n<p>Commit to a physical or mental well-being practice<\/p>\n<\/li>\n<li>\n<p>Attend an approved Wellness or HR activity<\/p>\n<\/li>\n<li>\n<p>Read one approved wellness article<\/p>\n<\/li>\n<\/ul>\n<p>When you\u2019re done, submit the <a href=\"https:\/\/docs.google.com\/forms\/d\/e\/1FAIpQLSfJgcfZP1dHq5ZzVp6HJjqiLzbS1mHzz-LfehEfAUuvMM-fqA\/viewform?usp=header\"><strong>Google Form<\/strong><\/a><\/p>\n<p>Questions?
