There are no excuses for late registrations without communication.<\/p>\n<div>\n<div>\n<div>\n<div><span class=\"il\">Breakdown<\/span>\u00a0of what needs to be completed for the internship registration:<\/div>\n<div>\n<div>\n<ul>\n<li>Data &amp; Learning Objectives Form \u2013 completed by you and your internship supervisor at your internship site<\/li>\n<li>Academic Agreement Form \u2013 completed by your faculty coordinator (dependent on major, not\u00a0<em>always\u00a0<\/em>the same as your faculty advisor)<\/li>\n<li>Internship Agreement Form \u2013 completed by you<\/li>\n<\/ul>\n<p>You will need to upload all 3 of these forms, completed, separately, to the Etrieve submission.
